The Tenderer has to Submit the Following Documents for e-GP Tender:
1. Similar Works Online Trade License & Payment Slip 2024-2025 Trade License must be Identical to the Related Works.
2. Online TIN Number Certificate.
3. Income Tax Payment Certificate Fiscal Year 2024-2025.
4. Income Tax Certificate, Income Tax Payment Certificate (2024-25) with Tax Assessment Return Slip, Tax Assessment Journal and Must be submitted 3 Year Income Tax Turnover.
5. VAT Registration Certificate (Procurement Provider), Online VAT Return Submitted for Last 03 (Three) Months & Update VAT Honour Card Last Two Years. (VAT, TIN Registration & Trade license same address mandatory).
6. Update Drug License Certificate (Only Medicine Group).
7. Copy of National Identity Card (NID) of The Proprietor.
8. Copy of Citizenship Certificate of the Proprietor.
9. Updated (2024) Certificate of The Member of the Chamber of Commerce & BMIHEDMA with Payment Slip.
10. Updated Notarized Affidavit of Sole Proprietorship declaration in case of sole Proprietorship. [Not signed before 01.07.2024]
11. Certificate of Non- Bankrupt and Non-Blacklisted. [Not signed before 01.07.2024]
12. Bidders must submit bids under SR rate. Abnormal rates are not acceptable.
13. The Procuring entity reserves the right to accept or reject any or all Tenders Without assigning any reason Whatsoever.
14. 100% sample of item Listed in the Schedule must be Submitted Before Tender Closing Date as per kept in Upazila Health Complex Office Store. At least 70% of the Samples Must Be Approved by the Sample Approving Committee. If less than 70% samples Unaccepted/ Unapproved by the Sample Approving Committee, the Tender will be Rejected Without any Reasons. Each Item number must be Numbering/Quoted on the Sample as per schedule. [Sample Receipt Copy Must Be Submitted Otherwise the Bidders Will Be Declared Non-Responsive.]
15. According to Section 98 (23) of PPR-2008, if a Bidder Submits such Abnormal Bid, he shall submit Evidence (e.g, Agreement Under Non-Judicial Stamp) in his favor. At the Same Time, an Undertaking has to be Submitted with the PE to Supply the Product at the Specified Time and at the Specified Price while Maintaining the Quality at that Price.
16. Original Manufacturer’s Authorization letter & Certificate of Medicine Manufacturer’s Company ISO/GMP/US-FDA if Any (Medicine Group).
17. Attested Copy MRP Price List of Medicine Company of items with Submitted medicine should be Highlight Marker Pen.
18. Attested Copy of Specimen Signature Color Photo by a 1’st Class Gazetted Officer with Name Seal & Mobile Number Otherwise the tender will be Considered cancelled.
19. Along with the tender, the bidder must comply with the following conditions:
a. I have submitted the tender in compliance with the stated conditions.
b. I will not attempt to influence the tender verification, evaluation, or decision-making process.
c. I will not engage in any unethical work or behavior with the authorities. In this regard, an undertaking must be filed with the PE, including full name, address, and seal, on the institution’s official letterhead. All bidders must type this pledge on a computer and submit it as an affidavit through a Notary Public on a non-judicial stamp of Tk. 300/- (three hundred) & Undertaking that no Influence/ Lobbying/ Recommendation will be made by any Eminent Person/Persons for Getting the Contract will have to be Submitted. Otherwise, the tender will be considered canceled. As per Sub-Rule 3-(Tha) of Rule 4 and STD’s Section 1.E(34.1), the affidavit must be submitted to the Procuring Entity (PE) before the deadline for tender submission. (Attach the bidder's pledge as per the provided format.)
20. If TEC Want to Examine Submitted All Original Papers, Tender is bound to Submit (Refundable).
21. All Paper’s Submitted with Tender Proposal must be Updated & Must be Color Scanned Copy Otherwise the Tender will be considered Cancelled.
22. Submitted all Documents will be cheeked direct or online mailing with Tender. In Case of Submission of any Fraudulent Document, Tender will be Rejected and Tenderer or company will be directly Black-Listed by the authority without any Notice.
23. PE may take appropriate legal action against any person who violates the rules of PPA-2006 & PPR-2008, Files any complaint, disturbs public interest, or obstructs procurement by false information.
Note:1
1. Submitted documents will be cheeked online or direct mailing with manufacturer. In case of Submission of any Fraudulent Document, Tender will be rejected and tenderer or company will be directly Black-Listed by the authority without any notice.
2. Tenderer must quote medicine items brand manufactured by top 10 reputed Pharmaceutical Companies.
3. The price quoted in the tender must be under approved MRP/Official estimate rate. Abnormal rates will be considered void. MRP must be Marking with including the serial number of the medicine.
4. Where applicable, the product should have a quality control certificate from the concerned department/department or authorization of the importer/manufacturer and each packet/strip/ampule/vial shall bear the words "Government Property, Not for Sale" and shall have a red and green wrapper. The expiry date of the medicine should be at least 2 years from the date of work order.
5. Work order will be provided as per rule and bills will be paid subject to receipt of survey committee certification after receiving goods as per work order. Income tax and VAT will be deducted from the bill submitted by the supplier as per government rules.
6. If TEC want to examine submitted all original Papers, Tenderer is bound to submit (Refundable)
7. PE will take appropriate legal action over against any person who violates the rules of PPR -2006 and PPR-2008. Submitted documents will be checked online or by direct mailing with authority. If a tenderer submitted or attached any kind of false documents, then the tender will be rejected and the tenderer or company will be directly blacklisted by the authority without any notice.
8. Abnormal rates are not acceptable. In case of Default, the PE may Declare the Bidder Non-Responsive in Public interest.

The firm will be considered non-responsive if providing any additional documents those are not required. You Should Follow rules 48, 49, 55, 4, 97, & 127 PPR 2008 and its amendments 2009, 2010, 2016 and 2019. Submitted documents will be checked online or direct mailing with the manufacturer. In case of Submission of any Fraudulent Document, Tender will be rejected and the tenderer or company will be directly Black-Listed by the authority without any notice.
