The Tenderer shall submit with its e-Tender the following additional documents:

4. BIN Certificate 13-digit, Online VAT Return submitted for last Two Months & VAT Smart Card 2025-2026.
5. Up to date Drug License certificate (2025-26) (Applied for Medicine Group)
6. Bank Capacity Certificate & Bank Solvency Certificate.
7. Bank Statement from date of 01-01-2025 to 01-07-2025.
8. Photocopy of National ID of the proprietor with one copy photo.
9. Updated Certificate of the member of the Chamber of Commerce with payment slip.
10. BMIHEDMA Certificate with payment slip.
11. Updated Affidavit copy of proprietor’s ownership by notary public on non-judicial stamp after 01-07-2025.
12. Updated Certificate of Non-Bankrupt, Non-Blacklisted and PPR Rules 49(Ga) by 1st class magistrate after 01-07-2025.
13. Three Specimens signature by a 1st class gazetted officer with name & seal otherwise the tender will be considered cancelled.
14. The required reports on financial standing, such as profit and loss statements and audited balance sheet shall be for the last Three years.
15. A certified photocopy of the ownership deed if the contractor's office is in his own building or the contract deed on the rent stamp of 300 in case of a tenant, which should be valid for at least 2 (two) years from the filing of the schedule.
16. A declaration to the effect that "all the documents submitted along with the tender are correct and no fake documents have been supplied" must be submitted his own pad.
17. VAT and income tax will be deducted from the bill according to the government rules. After paying the bill, if the government rate of deduction of income tax, VAT, etc. changes or if the deduction is less, the contractor will have to file an undertaking in his pad that he will pay later.
18. If TEC wants to examine submitted all original Papers, Tenderer is bound to submit (Refundable).
19. Manufacturer’s Authorization Latter is required for item of goods.

22. According to Public Procurement rules 4, 48, 49, 97 & 127 the revised PPR of Public Procurement Act 2006 and Public Procurement Act 2008 announced by the Government of Bangladesh, all rules and conditions related to tenders shall be applicable to this tender
23. 100% Sample/Catalouge of items listed in the schedule must be submitted before tender closing date as per kept in Upazila Health Complex Delduar, Tangail Store. At least 70% of the samples must be approved by the Sample Approving Committee. If less than 70% samples are unaccepted/ Unapproved by the sample approving committee, the tender will be rejected without any reasons. Each Item number must be numbered/quoted on the sample as per schedule.

11. Bank Statement of last 6 months.
12. BMIHEDMA Certificate with Money receipt.

14. An undertaking that no influence/lobbying/recommendation will be made by any eminent person/persons for getting the job will have to be submitted through the company letter head pad.
15. The Procuring entity reserves the right to accept or reject any or all Tenders without assigning any reason whatsoever.
