The Tenderer shall submit with its e-Tender the following additional documents:
1. Update 2025-2026 financial year MSR Trade License should be submitted.
2. Income Tax Registration Certificate should be submitted.
3. 2024-2025 Assessment Year Income Tax Payment Certificate & Assessment Journal should be submitted.
4. 2024-2025 Income Tax Return Slip should be submitted.
5. 13 Digit BIN Registration Certificate should be submitted.
6. Tenderer must submit previous 3 years Audit report from a reputed Audit Farm.
7. Certificate of VAT payment/VAT Honor Card for the financial year 2024-2025 should be submitted.
8. Last 3 (Three) month's VAT payment Document should be submitted.
9. 2025-2026 Financial Year Update Chamber of Commerce Inclusion Certificate with money receipt should be submitted.
10. Notarized sole proprietorship declaration in case of sole proprietorship and copy of all documents in RJSC certificate of formation of limited company in case of limited companies should be submitted.
11. Copy of the notarized declaration stating that the institution is not blacklisted anywhere should be submitted.
12. Authorization letter must be submitted from Manufacturer/Local Distributor in the specified format of e-GP.
13. Attested sample signature along with national identity card and photo of the tenderer should be submitted.
14. If a tenderer submits any kind of false information or if the attached paper does not get the verification report on time then the tender evaluation committee can make the tender non-responsive.
15. If TEC want to examine submitted all original Papers, Tenderer is bound to submit (Refundable).
16. Abnormal rates are not acceptable in other MSR groups.
17. According to section 98 (23) of PPR-2008, if a bidder submits such an abnormal bid, he will have to submit evidence in his favor. At the same time, a pledge and Sample has to be submitted along with the PE to deliver the goods at the specified time and at the specified price while maintaining the quality at that price.
18. If a tenderer chooses to submit a tender through a representative or authorized to sign on behalf of the organization, it must be notarized with a non-judicial stamp of Tk.300 containing the details and photograph of the representative.
19. This agreement will remain in force till 30 June 2026. Up to this time, the amount of work order will be given, the said goods will be bound to be delivered within the specified period, even if the contract period is extended up to three months in the public interest, an undertaking must be submitted in his own Letterhead Pad.
20. Along with the tender, the bidder must comply with the following conditions:
a. I have submitted the tender in compliance with the stated conditions.
b. I will not attempt to influence the tender verification, evaluation, or decision-making process.
c. I will not engage in any unethical work or behavior with the authorities. In this regard, an undertaking must be filed with the PE, including full name, address, and seal, on the institution’s official letterhead. All bidders must type this pledge on a computer and submit it as an affidavit through a Notary Public on a non-judicial stamp of Tk. 300/- (three hundred). Otherwise, the tender will be considered canceled, As Per STD’s Section 1.E(34.1), the affidavit must be submitted to the Procuring Entity (PE) before the deadline for tender submission. (Medicine Group Drawings Attach the bidder's pledge as per the provided format.)
21. PE may take appropriate legal action against any person who violates the rules of PPA-2006 and PPR-2008, files any complaint or disturbs public interest or obstructs procurement by false information.
Note: Submitted documents will be cheeked online or direct mailing with Authorities. In case of Submission of any Fraudulent Document, Tender will be rejected and tenderer or company will be directly Black-Listed by the authority without any notice. Please be carefully submit your tender. |

22. 100% samples of the items listed in the schedule shall be submitted before the last date of tender Submission as per maintenance in the hospital store. At least 70% samples shall be approved by the Sample Approval Committee. If less than 70% samples are unacceptable/not approved by the Sample Approval Committee, the tender shall be rejected without any reason. Each item number shall be numbered/quoted as per the schedule in the sample. Copy of sample receipt shall be submitted. Otherwise it shall be considered non-responsive by the evaluation committee. Samples shall be submitted for all MSR groups except medicines and furniture (furniture catalogue shall be submitted)
