10 Notarized declaration of non-bankrupt or non blacklist
11 Original Catalogue/Brochure for all items with Certificate of Country of Origin & CE/FDA-ISO Certificate (if any)
12 Manufacturer Authorization Letter for all items

19 Copies of experience certificate of satisfactory completion (Work completion certificate with the list of supplied products) of supply of similar goods within the last five (5) of years. i.e., years counting backward from the date of publication of IFT in the e-GP System
20 Supplied products must comply 100% of the published specifications, otherwise non-responsive (Comparative Specification Sheet).
