13. Notarized of Non-Bankrupt and Non-Blacklisted Updated this financial year by 1st class Magistrate must be Submit [Signed after 01-10-2025]
14. Three specimen signature attested by a 1st class gazette officer with name & seal and Number otherwise the tender will be considered cancelled.
15. The required reports on the financial standing, such as profit and loss statement and audited balance sheet shall be for the last 03(Three) Years from a reputed Audit Firm with DVS.
16. The contractor has to submit 01 (one) MSR Completion Certificate (Status-Completed/Ongoing) Verified by PE of e-GP system for supply of MSR material to any hospital under DGHS within last 03 (Three) F/Y years.
17. 100% Sample must be submitted to the PE office before the deadline of submission. Received copy of Submitted sample item must be submit in E-GP system. If the quality of the sample is not considered appropriate by the evaluation committee, it will be considered as nonresponsive.

NOTE:
1. If Tender Evaluation Committee (TEC) want to examine all original documents, Tenderer is bound to submit (Refundable).
2. Submitted documents will be checked online or by direct mailing with authority. In case of submission of any fraudulent document, Tender will be rejected and tenderer or company will be directly blacklisted by the authority without any notice.

The minimum specific experience as Supplier in supply of similar Goods (Linen) of at least 03 (Three) contract(s) successfully completed in the last 03 (Three) years in a signle work order, each with a value of at least Tk. 4,48,000.00 (Four Lac Forty Eight Thousand) per year in any Govt. Hospital with NOA, Contract Agreement, Work Order and Satisfactory Complection Certifications. [ i.e. years counting backward from the date of publication of IFT in the e-GP System.]
