The minimum specific experience as Supplier in supply of similar Goods (Chemical Reagent) of at least 02 (two) contract(s) successfully completed in single work order within the last 05 (Five) years, with a value of at least Tk. 16,00,000.00 (Taka Sixteen Lac) only per year in any minimum 300 bed or above Govt. Hospital/Institute with NOA, Contract Agreement, Work Order and Satisfactory Completion Certifications.[Years i.e. years counting backward from the date of publication of IFT in the e-GP System]. |

| TDS/PDS--D. e-Tender Preparation--

The Tenderer shall submit with its Tender the following additional documents:
1. Up to date Valid MSR Trade License 2025-2026 with payment Slip.
2. E-TIN certificate, Income Tax certificate 2025-2026 with return copy.
3. 13 Digit BIN Certificate, Last month BIN return copy, Honour Card valid till 30.06.2026 & VAT Clearance Certificate of 2024-25 issued from Customs, Excise and VAT Commissionerate Office.
4. Attested copy of National Identity Card of the tenderer.
5. Attested Three specimen signature and one PP photo of the tenderer must be submitted.
6. Updated 2025-26 Chamber of Commerce Certificate and Payment Deposit Receipt.
7. Tenderer must submit Last 05 years Audit report from a reputed Audit Firm.
8. Import Registration Certificate.
9. Updated Notarized Affidavit of Sole Proprietorship declaration in case of sole proprietorship or RJSC certificate in case of Limited Companies. [Not Signed before 01.01.2026]
10. Updated Notarized declaration stating that the institution is not non bankrupt and not-blacklisted anywhere. [Not Signed before 01.01.2026]
11. Original Manufacturer's Authorization Letter in the specified format of e-GP and GMP/ISO/US-FDA Certificate must be submitted from the company from which the tenderer will supply the reagent. Otherwise, the tender will be non-responsive.
12. Evaluation committee preserve the authority to declare null and void any tender proposal without mentioning any cause. Evaluation committee is not bound to accept lowest rate proposed. No tenderer is allowed to ask for any query in case of rejected tender proposal declaration in company pad. PE is not bound to purchase all items of schedule.
NOTE:
1. If Tender Evaluation Committee (TEC) want to examine all original documents, Tenderer is bound to submit (Refundable).
2. Submitted documents will be checked online or by direct mailing with authority. In case of submission of any fraudulent document, Tender will be rejected and tenderer or company will be directly blacklisted by the authority without any notice.
3. If any tenderer dose not fulfill properly the Tender Information Form (e-PG3-2), tenderer will be considered as non-responsive.
4. PE may take appropriate legal action against any person who violates the rules of PPA-2006, PPR-2008 & PPR-2025, disturbs public interest, or obstructs procurement by false information.
5. Work order will be provided as per rule and bills will be paid subject to receipt of survey committee certification after collection of goods as per work order. Income tax and VAT will be deducted from the bill submitted by the supplier as per government rules.
6. If a tenderer submits any kinds of false information or if the attached papers verification report does not reach in time, then the Tender Evaluation Committee (TEC) can make the tender non responsive.
7. This agreement will remain in force untill selection of next tenderer . Up to this time, the amount of work order will be given, the said goods will be bound to be delivered within the specified period, even if the contract period is extended up to three months in the public interest, an undertaking must be filed in its own pad.
8. The Authority reserves the right to reject or accept any or all tenders without assigning any reason.
9. Tenderers are kindly requested to provide all the documents correctly as stated above and asked not to submit unnecessary or excessive documents.
