The Tenderer shall submit with its Tender the following additional documents:
1. Update/ renewal medicine E-Trade License for 2025-2026.
2. Trade License Payment Deposit Receipt for last five years must be submitted.
3.Drug License group Ka wholesale must be submitted,
4. Income Tax Certificate for 2024-25 With return slip of 2024-25, 2023-24, 2022-23.
5.13 Digit BIN Registration Certificate & smart card with last 03 month's Online VAT Return Invoice Form should be submitted.
6. Update Chamber of Commerce certificate with money receipt 2025-2026.must be submitted.
7.A-Update Tenderer Product Quality Standard Certificate (ISO/BSTI/CE). must be submitted.
7.B-Update Import Registration Certificate (IRC) with Import certificate (not less than 1 Cr) for Madicine or Medical Instrument & Chemical Reagent for last 03 years (2022-23, 2023-24, and 2024-25) must be submitted.
8.A-Recent color photograph, NID, specimen signature & citizenship certificate at company letter head pad of authorized person.
8.B-All papers uploaded must be scan copy or attested photocopy from the main copy otherwise it might be rejected due illegibility. All documents must clearly bear the signature, seal, and mobile number of a 1st class gazetted officer.
9. Affidavit of ownership of the contracting company by the 1st class magistrate must be submitted.
10.Shall submit Clause by Clause comments on GCC and PCC Note:a. To avoid unwanted situations, it is requested to take enough time to submit tender documents without waiting till the last time
b. For justification purposes, all submitted documents must include proper contact information, including email address and contact number,
11. Audit report (DVC) 03 years must submitted.
12. Bidders must submit bids for Top Ten Medicine Company in Bangladesh.
13. Medicine Company Authorization with GMP/FDA/ISO Certificate Should must be submitted by Tenderer in case of Medicine.
14. MRP chart of Medicine Company of required items should be highlighted by marker pen.
15.A certified photocopy of the ownership deed if the contractor's office is in his own building or The contract deed of rent on non-judicial stamp of 300/-in case of a tenant, which should be valid for at least 2 (two) years from the filing of the schedule.
16. There is no case of sedition in the name of the supplier organization or the proprietor of the organization in the court and a declaration has to be submitted through a non-judicial stamp of Tk. 300/- so that the organization is not blacklisted. must be submitted.
17.A-Declaration to the effect that "all the documents submitted along with the tender are correct and no fake documents have been supplied" has to be submitted through a non-judicial stamp of Tk. 300/-Submitted documents will be checked online or by direct mailing with authority. In case of submission of any fraudulent document, tender will be rejected and tenderer or company will be directed blacklisted by the authority without any notice.
18.A-A bank balance of (01) one crore taka.must be maintained from the date of publication of the tender till the day before the tender opening.
18.B Bank Solvency Certificate with bank statement. Bank Statement of last 6 months up to the date of tender publication where his Satisfactory Transection is Taka 1.5 Crore.
19. VAT and income tax will be deducted from the bill according to the government rules. After paying the bill, if the government rate of deduction of income tax, VAT, etc. changes or if the deduction is less, the contractor will have to file an undertaking in his pad that he will pay later.
20. In the case of any goods covered by the tender schedule, the supplier must submit on his own pad a commitment to provide after-sales service for at least 2 years from the day of delivery, in accordance with the demand of the authority
21. As a proof of the quality of the medicine, the appropriate certificate (quality control certificate) batch-wise of the medicine should be submitted at the time of supplying the medicine undertaking own his pad.the authority to increase or decrease any amount of scheduled item, even may decide not to buy any item asked for proposal on the basis of hospital demands. No tenderer is allowed to ask for any quarry in case of rejected tender.
22. Evaluation committee preserve the authority to declare null and void any tender proposal without mentioning any cause. Evaluation Committee is not bound to accept the lowest rate proposed. PE preserve the authority to increase or decrease any amount of scheduled item, even may decide not to buy any item asked for proposal on the basis of hospital demands. No bidder can request any compensation for any losses in the event of a rejected bid.
23. Bidders must submit bids lower than MRP. Tenderers must submit their company name with serial number in the price and delivery schedule form.
24. All original paper is bound to submit if TEC want to examine. (Refundable).
25. Samples as per instruction given on Technical Specifications and Compliance of Goods and related services (Form e-PG2-4) shall have to submit as well as received copy of submitted samples list shall have to upload with other documents on e-gp system.
26.N: B: Bids that are either excessively higher or lower than the official estimated value-beyond the percentage allowed by regulation-will be rejected. (According to PPA 2006 Clause 32-KA-A-E) Revised PPR 2025.

6. Updated Chamber of Commerce inclusion with deposit slip and Update BMIHEDMA Certificate with money receipt.
7. Company Profile A-Update Tenderer Product Quality Standard Certificate (ISO/BSTI/CE). must be submitted.
8.B-Update Import Registration Certificate (IRC) with Import certificate (not less than 1 Cr) for Madicine or Medical Instrument & Chemical Reagent for last 03 years (2022-23, 2023-24, and 2024-25) must be submitted.

10.Sole Proprietorship/Ownership declaration signed by 1st Class Magistrate on Non-Judicial stamp of Tk. 300.00 or copy of all document in RJSC certificate of formation of limited company in case of limited companies (Not signed before 01/07/2025).

12.The required reports on the financial standing, such as profit and loss statements sheet for Last three years. FY 2024-2025 profit and loss statements sheet must have with Documents verification code (DVC) number. Update credit rating report must be submitted (Both are separate documents).
13. (a)All bidders must type pledge on a computer and submit it as an affidavit through a Notary Public on a non-judicial stamp of tk 300/- (three hundred). Otherwise, the tender will be considered canceled, the affidavit must be submitted to the Procuring Entity (PE) before the deadline of tender submission and received copy must be submitted on e-GP System. Otherwise Tender will be declared as non-responsive.
14.(b)A certified photocopy of the ownership deed if the contractor's office is in his own building or the contract deed on the rent stamp of 300 / - in case of a tenant, which should be valid for at least 2 (two) years from the filing of the schedule (Not signed before 01/07/2025).
