The Tenderer shall submit with its e-Tender the following additional documents:The Tenderer shall submit with its e-Tender the following additional documents:

1. National Identity Card of the bidder.

2. Updated Income Tax Payment Certificate and Return, which must be certified to have paid income tax properly. (No certificate in process or pending will be eligible to be accepted).

3. Updated VAT Registration Certificate BIN (13 Digit).

4. Updated renewed Trade License, Group based.

5. Photocopies of all the documents submitted with the tender documents shall be attested by the Gazetted Officer of 1st class. Failure to submit such documents will result in cancellation of the tender. Mobile number and Code number of attesting officer should be mentioned.
6.In case of organization, the applicant has to submit attested copy of affidavit of ownership issued by 1st
class magistrate"

7. Submission the Update - Chamber of Comerce , Medical Instruments & Hospital Equipement , Bangladesh Scientifice Instruments Dealers Association Membership Certificate With all Money receipt

8. It is mandatory to submit the sample as per the demand, however responsible (successful) bidders will be ask to provide samples.
9. All Submitted Documents should have signature of the tenderer.
10. In case of higher rate than SR Book of DGHS, will not be eligible for this tender.
11. All product should be from reputed manufacturer.
