The satisfactory completion of supply of similar Goods of minimum Tk. 150,00,000.00 (One Crore Fifty Lac) only under maximum 2 (One) contracts in the last 3 (Three) of years. i.e. years counting backward from the date of publication of IFT in the e-GP System.

1. Update Group wise Trade License with Payment slip, Income Tax Certificate with TIN certificate and Return Slip with Income Tax Assessment, BIN certificate with Return Certificate.
2. The Specific experience certificates shall be validated with the completion certificate (substantiated by IFT/Advertisement, NOA, and Contract agreement of similar goods in supply of Government/Semi Government /Autonomous Organization with work order & contract).
3. Original Manufacturer’s Authorization Letter is required for item of goods. Any third party authorization will not be accepted.
4. Original Catalogue/Brochure/ MRP Chart of Medicine Company of items with submitted medicine should be Highlight Marker Pen.
5. Import Registration certificate submitted.
6. Liquid assets certificate, NOA, Contract agreement & work completion certificate must be color scanned copy.
7. Affidavit of ownership of the contracting company by the First Class Magistrate must be submitted if Proprietorship Company or Certificate of Ownership if Limited Company.
8. Update Chamber of Commerce Certificate with Payment Slip.
9. Update Drug License with No Arbitration.
10. Recent Color Photograph, NID and Specimen Signature at Company Letter head Pad of Authorized Person.
11. GMP/DGDA certificate must be submitted.
12. 3 (Three) Years Audit Report in the last 5 (Five) Years.
11. BMIHEDMA Certificate with Money receipt.
12. Certificate of Non-Bankrupt and Non-Blacklisted.
13. Sample/Catalog hardcopy of all items must be submitted to PE office before deadline of submission of tender.
 14. Must have two consecutive years of supply experience in the same government health institution.
15. Tenderer must visit hospital lab to see machine & need Same machine Manufacturer Authorization Letter.
